What is Holter Monitoring ?

Holter Monitoring

Holter monitoring is a technique of continuously monitoring the electrical activity of a patients heart using a special device known as a Holter monitor. It is somewhat similar to an electrocardiogram, however,in this case, the monitoring is done for 24 hours or even more. This involves sticking tiny electrode patches on the patients chest and abdomen, which are connected to a small, portable machine using leads.

Holter monitoring makes it responsible to measure and record electrical impulses generated by the heart to detect any unusual change and abnormality, which could be linked to an underlying heart condition, such as stress-induced arrhythmia.

Why does one need Holter Monitoring?

Holter monitoring may be recommended to assess and determine the underlying cause of occasional chest pain and other symptoms like breathlessness, syncope and lightheadedness, in cases where other modalities like TMT don't work.

It may also be recommended for:

  • Identifying Irregular Arrhythmia: Holter monitoring can be used to identify and diagnose arrhythmia that is triggered occasionally due to factors like stress
  • Assessing the working of a Pacemaker: Patients who have undergone pacemaker implantation may be advised to go for Holter monitoring to determine whether the device is functioning optimally.
  • Tracking the Prognosis of Patients: Holter monitoring may be used to check how well a patient is responding to the treatment for severe arrhythmias.

Types of Holter Monitoring

Holter Monitoring is broadly classified and based on the technique that is used:

Wireless Holter Monitors

The monitors have wires to connect them to the patient’s chest, however, the data is transmitted to the doctors online via cell phones.

Event Monitors

The monitors do not record heart rhythm continuously but only when any symptoms are being experienced

Post-Event Recorders

These are quite small, and often worn as a wristwatch. They are to be activated manually once the symptoms start

Presymptom Memory Loop Recorders

These are as small as a cell phone. These continuously monitor the heart rhythm but do not store any data unless you press the button.

Autodetect Recorders

These do not need to be started and function continuously. They automatically record the activities without any command.

About Holter Monitoring Procedure

Preparation

  • If your doctor feels that you need Holter monitoring, you will be briefed about the procedure in detail to make sure that you are well aware of what to expect.
  • It is important to wear loose and comfortable clothes. Tight clothing can make it difficult to place the adhesive patches on the chest.
  • It is advisable to take a shower before the test as you won't be able to do so for the next 24 hours or more, depending on how long you have been advised to wear the monitor.
  • You should refrain from using any lotion, oil or cream on your torso area as this makes it difficult to stick the electrode pads.
  • If you have been taking any medication lately, it is important to make sure that your doctor knows about all of them.
  • Keep track of your daily activities by maintaining a record.

What does the Procedure involve?

  • Holter monitoring is a non-invasive procedure, generally performed on an outpatient basis, which means that you will be discharged on the same day.
  • Doctors carefully place the electrode patches on your chest and abdomen. The monitor can be worn like a bag or clipped to your belt. Once the device is hooked, it will be turned on.

What follows the Procedure?

  • After the procedure, you will be required to keep the device on at all times, whether you are standing, sitting, sleeping or indulging in any activity
  • While the device is on, you need to stay away from electrical appliances like mobile phones, televisions and electric blankets, as these can disrupt the normal results
  • You need to avoid getting wet at all costs, which means that you cannot even take a shower while the device is on
  • If you develop any unusual symptoms, you should immediately connect with your doctor and go for proper medical intervention
